Digital Waiver

Waiver of Liability and Photo Release for Warrior Blood Jiu-Jitsu

All students must sign this waiver of claims and liability. By signing this waiver of claims and liability the student, parent or guardian acknowledges that they have read this agreement and will abide by all rules of Warrior Blood Jiu-Jitsu. Signing this waiver also means that the undersigned/student releases Warrior Blood Jiu-Jitsu and its instructors from any liability in case of injury or death from the instruction at Warrior Blood Jiu-Jitsu.

In consideration of being allowed to train at Warrior Blood Jiu-Jitsu the undersigned/student acknowledges that grappling, kickboxing and/or participation in any physical activity may result in serious injury including death, and or other damage, due not only to the undersigned/students actions, inaction or negligence, but also to that of others associated with and or present at these or other classes.

Furthermore, I acknowledge that there may be risks not known to me and or not reasonably foreseeable at this time. I personally, totally and completely assume all risks involved or in any way related to my participation in any of the activities hosted by Warrior Blood Jiu-Jitsu instructors or facilities.

The undersigned/student, heirs, executors and successors hereby waive any claim of accidental and or negligent tort damages or injury against Warrior Blood Jiu-Jitsu, its instructors and any physical location where a Warrior Blood Jiu-Jitsu class is taught.

I grant to Warrior Blood Jiu-Jitsu, its representatives and employees the right to take photographs of me and my property in connection with Warrior Blood Jiu-Jitsu and marital arts. I authorize Warrior Blood Jiu-Jitsu, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Warrior Blood Jiu-Jitsu may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
